BLUE NILE CAFE
Address: 933 E. University Drive, Tempe
Major violations: 10
Inspector’s comments: Not all certified managers and food handlers had a food handler’s card; some menu items may have contained raw or undercooked meat; food was stored in dirty milk crates on the floor; raw eggs were stored above other foods in refrigerators; cooked rice in the walk-in refrigerator was not dated; cooked meat and vegetables in the refrigerator was outdated and covered in mold, frijoles (beans) and cooked vegetables were outdated; a food thermometer was not being used to monitor food temperatures; the exhaust hood and the top shelf in the kitchen was dirty; the kitchen walls, floor and ceiling were dirty; employee handled ready-to-eat food with bare hands; pots on the storage shelf were dirty with rings of food debris.

PACIFIC CAFE
Address: 7272 E. Indian School Road, Scottsdale
Major violations: 7
Inspector’s comments: Can opener blade and mixer splash zone was dirty; employee touched trash can then returned to prepare food without washing hands; two large cans of soup under the counter were rusty and soiled; food contact surfaces were not cleaned and sanitized prior to use.

Address: 7316 E. Indian School Road, Scottsdale
Major violations: 7
Inspector’s comments: Kitchen knives at the prep table were dirty; soda holsters at the main bar had slime buildup; several ready-to-eat foods in the reach-in cooler had no discard dates; one of the restrooms had a water leak; can of insecticide was under the cabinet at the main bar; two bottled drinks stored in ice bins, ice not protected from cross-contamination.

TACO BELL #4344
Address: 2663 E. Broadway Road, Mesa
Major violations: 6
Inspector’s comments: Employee put on gloves without washing hands first; chemical sanitizer not at proper concentration and/or hot water for sanitization; live roach on the drain board of the sink; cooked potato bites sitting out without a discard time labeled on the container; not all employees had food handler cards.

FLO’S ASIAN KITCHEN
Address: 16495 N. Scottsdale Road, Scottsdale
Major violations: 5
Inspector’s comments: Employee started working without washing hands first; employee wiped hands on apron and continued preparing food; food contact surfaces not cleaned and sanitized prior to use; spray bottles of chemicals stored in cabinets with single-serve items; case of straws separated chemicals from food and food-contact items.

SUEDE RESTAURANT & LOUNGE
Address: 7333 E. Indian Plaza, Scottsdale
Major violations: 4
Inspector’s comments: Delivery systems for bars not functioning properly; live roaches in reach-in cooler; 2 pounds cream cheese had severe mold growth.
