Address: 1805 E. Elliot Road, Tempe
Major violations: 7
Inspector’s comments: Waitress/bartender used bare hand to pick up lemon wedges for drinks; cook guided french fries onto plates with bare hands; food contact surfaces and equipment not cleaned frequently and properly to prevent food contamination; no sanitizer in the final rinse at the dishwasher; cook line’s equipment greasy and filthy; outdated sliced ham still in refrigerator; wall behind grill and fryer dirty and greasy; nasal spray on bar next to coffee maker, hand lotion next to spoons and forks.

LOS TRES AMIGOS MERCADO Y CARNICERIA

Address: 253 E. Broadway Road, Mesa
Major violations: 6
Inspector’s comments: Beer can, soap dispenser, and wet wiping cloth stored in hand sink; salsa and deli ham stored under raw meat in deli case; meat slicer and fryer baskets dirty; deli meats and cheeses in deli case not dated and cooked beef in walk-in cooler not dated; live roaches in kitchen and meat market; employee at front desk smoking as they entered the building.

Address: 1335 W. University Drive, Tempe
Major violations: 6
Inspector’s comments: Cooked, ready-to-eat fried rice and cooked, ready-to-eat noodles were not held at proper temperature; employee hand sink filthy with grease and food debris; raw chicken was stored above ready-to-eat carrots and tofu in cooler; knives on the back storage rack were filthy with dried food debris and grease; containers of fried chicken in cooler were not dated; floors and storage area have an accumulation of soil residues; can of Raid was found in the back storage closet.

LILY BOWL
Address: 785 W. Warner Road, Gilbert
Major violations: 5
Inspector’s comments: Eggs stored above fried chicken and bean sprouts stored by raw chicken; food debris on vegetable cutter, veggie peeler and residue accumulated on containers; shelf and refrigerator outside of freezer dirty; grease buildup on the floor under cook line, under equipment and by the deep fryer; floor under equipment by the floor sink and wall in the kitchen area dirty; employee personal items stored with business goods in the kitchen area; prep table, cutting board, knife, equipment had food debris; a food container was stored in mop sink; potatoes stored on the floor in the kitchen.

Address: 1705 E. Broadway Road, Tempe
Major violations: 5
Inspector’s comments: Dish of unwrapped toothpicks at the front counter (toothpicks are required to be individually wrapped or in a dispenser that exposes them one at a time); food handlers handled ready-to-eat foods with their bare hands; cooked vegetables were outdated; the menu had raw fish on it and lacked the required identification of which items contained raw fish; big menu lacked the required warning statement; bags of food were on the kitchen floor; raw eggs were stored above cooked food in the walk-in refrigerator.
