Address: 3135 S. 48th St., Tempe
Major violations: 8
Inspector’s comments: Cooked noodles and black beans were not kept at proper temperature; food service workers were not preventing contamination of ready-to-eat food; back sink was filthy with dried food debris and grease; raw chicken and raw shrimp were stored over ready-to-eat vegetables in the cooler; both the back and front door to the kitchen area were filthy with dust, grease and food debris; can of bug killer was found in the back office area; makeshift utensil used for straining oil from food was held together with duct tape and wire; can opener and two vegetable peelers were filthy with dark grease and rotting food debris; two live roaches were found in establishment.

Inspector’s comments: Employee drinks in reach-in cooler did not have a lid and straw; no paper towels at hand sink; raw beef patties stored over cooked beef in reach-in cooler; to-go boxes not properly stored; deli meat, cooked beef, cooked chicken were not dated; manager did not have certified food manager card; sink faucet not working properly; galvanized steel bucket used for cooking showed evidence of deteriorating interior metal coating; ventilation hood soiled with grease; broken toilet seat.

Major violations: 6
Inspector’s comments: Two bottles of half-empty vitamin water drink found under prep table; drill with spatula attached was used to stir salsa (not approved kitchen equipment); boxes of tomatoes and produce on the floor in the cooler; ice machine lid was not attached to ice machine and can fall on the floor, causing contamination; chili rellanos, tacos, cooked meats dated; grease traps at ventilation hood unit soiled with grease residue; not all employees had food service worker cards; chemical spray bottle unlabeled; employees washing hands at hand sink without soap, soap dispenser not working properly.

Inspector’s comments: Employee handled dirty dishes and immediately handled clean dishes and glasses without washing hands between these tasks; two milkshake spindles had pink mold; ice machine and container for ice scoop soiled and dusty; pan of chicken wings in walk-in freezer was not covered; boxes of pasta, pizza crust mix, and large bag of rice stored on floor; container of sugar did not have a cover and contained unidentifiable black particles; drain pipe for hand wash sink at cabana bar disconnected, causing wastewater to drain on floor; no back-flow prevention device (RPZ valve) for soda machine at cabana bar, establishment did not have records for yearly testing of RPZ valves; rodent droppings at indoor bar; no disclosure of potentially raw or undercooked items on menu.

Inspector’s comments: Cooked pasta not held at proper temperature in refrigerator; food on cooks’ line cold; raw shell eggs stored above butter in walk-in refrigerator; hand wash sink used for storage; utensils with dried food debris on them stored as clean; dispensing utensil not provided for lemons.
