Address: 1606 W. Baseline Road, Tempe
Major violations: 8
Inspector’s comments: Raw shrimp being breaded on cooks line directly before ready-to-eat foods being prepped in the same location; not all employees have food handler cards; service lids kept in rusted galvanized bucket and Styrofoam single to-go containers placed without wrapping face down on raw wood surface; employee with hands covered in white residual was seen placing french fries on plate to serve; some employees did not wash hands long enough.

O’KELLEY’S sports BAR & GRILL


Address: 2120 W. Guadalupe Road, Mesa
Major violations: 7
Inspector’s comments: An employee was smoking in the kitchen; soda guns at bar had slime; raw shell eggs stored over cooked chicken wings in the walk-in cooler; manager not certified and does not demonstrate applicable food safety knowledge; current food service worker cards are not on file for all employees.

Address: 9221 E. Baseline Road, Mesa
Major violations: 6
Inspector’s comments: Floors in the walk-in cooler and under the equipment soiled; personal medications not stored properly; chemicals not properly marked for spray bottles, oven cleaner bottle filled with Windex; employees’ bags and cell phones not properly stored; salt and pepper stored in unmarked containers; wet towels not sanitized.

SCOTTSDALE BBQ CO.


Address: 8471 E. McDonald Drive, Scottsdale
Major violations: 6
Inspector’s comments: Employee touched face with gloved hand then returned to preparing food; crushed tomatoes had mold growing inside can; toxic materials not properly labeled. Due to repeated, severe, and/or numerous violations noted on this inspection, legal action is being initiated.

CARNICERIA PANADERIA LA ESTRELLA


Address: 819 E. Broadway Road, Mesa
Major violations: 5
Inspector’s comments: Potentially hazardous foods stored in reach-in cooler near the prep area was not at proper temperature; hand wash sink had a trash can and buckets stored in front of it; soap not present at the outdoor cooking area; raw eggs and raw meat stored above ready-to-eat foods in the walk-in coolers; employees noted washing and rinsing pots and pans without sanitizing; bags of salt were stored on the ground in the dry storage area; general cleaning needed on the floors, walls and ceiling; personal items stored above the prep table.

SULLIVAN’S PIER


Address: 1902 E. Main St., Mesa
Major violations: 4
Inspector’s comments: Raw fish stored above ready-to-eat foods in the walk-in cooler; no certified food manager present in the establishment; half of the employees have food service worker cards; ceiling in the back storage area is falling down; vitamin pills stored on the prep table; water jugs without lids and straws used for the employee drinks in the food prep area.

I'm glad that they are cracking down on violations in walk-in coolers. This is a common area of food poisoning. Don't believe me? Check out http://www.kysorpanel.com Suggest removal of this comment
